It is called "outside parent" or"外亲"or“外親”.You can find that function at the lower left corner,in the "model manipulation" section,there is an "OP" or“外”button.Click that button will open up a window,and I think you can figure out how it works by just looking at that window.Basically it binds one of the bones in model A to one of the bones in model B,as if it is its parent bone.
You can register that bind at any frame during your mmd when needed,so those stuff can be placed elsewhere and not move with the model at the begining.

It might be easier for you to photoshop the texture file of the breast, so you simply replace the areola with the black heart in the texture picture file?

Beware that if you OP-register objects to self-swaying bones such as breasts, neckties or twintails, then will the added object hover in the air lagging behind that bone in an annoying manner upon rendering. (That's why I was forced to add the clothes peg on the necktie in PMXeditor instead of with MMD OP register in this video: https://ecchi.iwara.tv/videos/2k99rikkvfqma01b . The same was true for the clothes pegs on the nipples in this video: https://ecchi.iwara.tv/videos/lyz8qsozqf85qq3a . It looked really silly with clothes pegs hovering in the air like dragonflies...)
